Our History
Hazelwood is an Intermediate Care Facility supporting individuals with intellectual and developmental disabilities since 1971. Located in Louisville, Kentucky, Hazelwood opened in 1907 as a tuberculosis center. New Vista began operating Hazelwood in 2018 with the goal to assist people in developing skills and interests to help them reach personal goals and live meaningful, balanced lives.
