Our History

Hazelwood is an Intermediate Care Facility supporting individuals with intellectual and developmental disabilities. Located in Louisville, Kentucky Hazelwood opened in 1907 as a tuberculous center. Since 1971 Hazelwood has served the region as an intermediate care facility. New Vista Community Mental Health Center began operating Hazelwood in 2018 with the goal to assist people develop skills and interests to help them to reach personal goals and live meaningful, balanced lives. We offer services that focus on wellness, community living, work and leisure in a safe environment. In addition, Hazelwood provides employment opportunities on and off campus.

In addition to the variety of services on campus, we also assist families and agencies across the Commonwealth. In 2015, the Hazelwood Specialty Clinic opened with state-of-the-art treatment facilities for individuals with IDD in the areas of medical, dental, behavioral health, rehabilitation and therapeutic needs. The Hazelwood Specialty Clinic is a resource for the entire region utilizing the most current treatment practices and technology. Hazelwood is proud of the many partnerships with advocacy groups, universities and medical specialists that promote better services, care, understanding and treatment for individuals with IDD.
